September 24, 2003- Graco: New Safety Instructions to Prevent Injuries with Portable Play Yards with Raised Changing Tables
In cooperation with the U.S. Consumer Product Safety Commission, Graco Children’s Products Inc., of Exton, Pa., is providing new instructions for about 538,000 “Pack ‘n Play” portable play yards with raised changing tables. When children are placed in these portable play yards when the changing table is still in place, they can crawl under and lift the table up. If this occurs, a child’s head and neck can become trapped between the changing table and the play yard rail, causing a strangulation hazard. The recall is designed to warn consumers of this hazard, and to provide warning labels to affix to the changing tables.
October 10, 2001- Dorel Cosco Playpen Recall
In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Dorel Juvenile Group, of Columbus, Ind., is voluntarily recalling about 102,000 Cosco "Zip n Go," "Okie Dokie," and "Carters" playpens manufactured between May 1995 and December 1997. The plastic tabs on the playpen that lock the rails into the corners can break or loosen over time, allowing the rails to turn inward, collapse and entrap an infant.

In cooperation with the U.S. Consumer Product SafetyCommission (CPSC), Century Products Co., of Macedonia, Ohio, is voluntarily recalling about 50,000 Fold 'N Go Care Centers for repair. The care center's bassinet could have loose fabric, which can create a pocket near the floorboard in which an infant can become entrapped and suffocate.

In cooperation with the U.S. ConsumerProduct Safety Commission (CPSC), Evenflo Company Inc., of Piqua, Ohio, isoffering free hinge cover kits to all 1.2 million owners of its portableplay yards sold under the Happy Camper, Happy Cabana and Kiddie Campermodel names. If the hinges on the product are not fully rotated, theproduct can collapse, possibly trapping the child in the "V" formed by thefolded top rails. Additionally, if leaned or sat upon, the rotatingplastic hinges in the middle of the folding top rails can crack or break,presenting a sharp edge or possibly allowing the child toescape.

The top rails on these playards maycollapse unexpectedly, trapping a child at the neck in the "V"formed by the collapsed top rail.

The mesh on playards made betweenNovember 1995 and September 1996 may unravel from the seam, resultingin a possible choking or entanglement hazard.

In cooperation with the U.S. ConsumerProduct Safety Commission (CPSC), North States Industries ofMinneapolis, Minn., is offering a repair kit forapproximately 70,000 Superyard children's play enclosuresand 6,000 Superyard Extension Kits manufactured betweenNovember 8, 1994 and October 17, 1995. Small plastic lockingposts can be pulled loose when any of the enclosure's panelsare disconnected, presenting a choking hazard to smallchildren.
